Programs the library is missing
Every AI-designed plan is built from MAINLIBRARY_V3. When a client's evidence calls for a program the
library does not contain, the designer is required to name it here instead of forcing a near-miss program
into the plan or inventing an ID. That makes clinical use of the map the thing that grows the map.
Proposed — the model explicitly named a missing program and justified it against
evidence-based practice for that client. Orphan — the model returned an ID that is not in
MAINLIBRARY_V3; it was dropped from the clinician's plan before they ever saw it, and logged here because
the same invented name recurring across different clinicians is itself evidence of a gap.
Nothing on this page writes to the library. Approving sends the program through
manage_mainlibrary.php?action=add — the same gated, auto-backed-up write path everything else
uses, which enforces the five rules in MAINLIBRARY_WRITE_RULES.md (5-language titles, SD in EN+ES, target
limits). Sorted by how many times each gap has come up.
